- Stop treating an empty Tantivy search index as OK. Doctor emits search-empty (warn/index) with remediation ledgerful index and a non-OK Index Health line. Search captures pre/post document counts and surfaces search_index_status via BridgePayload::Insight so empty index is not collapsed into silent no-matches.
